If you need a Certificate of Insurance for an event, you must complete the Certificate of Insurance/Event Review Request form two weeks prior to your event.

If you need a Certificate of Insurance for your University, your mortgagee, or any one else requesting a Certificate of Insurance that is not associated with a specific event, you must complete the Non-Event Specific Certificate of Insurance Request form.
