If you are not requesting a Certificate of Insurance for a specific event, please complete this form instead. Be sure to review our Position Paper on Risky Activities to make sure your type of event is not on the risky activities list.
    Reminder: We cannot approve of events where chapter members are selling alcohol (e.g. fundraising activities at athletic events).
